Job Title: Lead Case Manager Profession: RN Specialty: Case Management Duration: 10 Shift: 08:00-16:30 Hours per Shift: 8 Experience: Minimum of 2 years of social work or case management experience in an inpatient setting preferred. License: Current Registered Nurse or Social Work licensure or a Master's or Bachelor's in Social Work or healthcare professional licensure. Certifications: Certification in Case Management or Rehabilitation preferred, such as: - Commission for Case Manager Certification (CCM) - Association of Rehabilitation Nurses (ARN) certification - American Case Management Association (ACM) - Board Certification in Case Management by the ANCC Must-Have: Basic computer knowledge Effective communication skills in English, both verbal and written Description: The role is dedicated to the treatment and recovery of individuals experiencing severe injury or illness. Rehabilitation programs provide ongoing care and specialized treatment to patients throughout their recovery journey.
