Job Description Job Description Overview We are seeking an
experienced and dedicated Home Health Clinical Manager to oversee
the delivery of high-quality home health services. This leadership
role involves managing clinical operations, ensuring compliance
with healthcare standards, and fostering a patient-centered
environment. The ideal candidate will possess strong medical
management skills, healthcare management expertise, and experience
with accreditation standards such as JCAHO. This position offers an
opportunity to lead a dynamic team committed to exceptional patient
care within a regulated healthcare setting. Duties Lead and
coordinate clinical operations to ensure efficient delivery of home
health services. Oversee patient care plans, ensuring they meet
regulatory and quality standards. Manage and supervise clinical
staff, including nurses and therapists, providing guidance and
performance evaluations. Ensure compliance with CHAP accreditation
requirements and other relevant healthcare regulations. Develop and
implement policies and procedures to improve clinical workflows and
patient outcomes. Collaborate with interdisciplinary teams to
facilitate coordinated care. Monitor quality metrics, audit
clinical documentation, and implement continuous improvement
initiatives. Serve as a liaison between administration, staff,
patients, and external agencies to promote effective communication.
Maintain up-to-date knowledge of healthcare laws, regulations, and
best practices in home health management. Requirements Proven
experience in medical management and healthcare management within a
home health or similar setting. Strong leadership skills with the
ability to manage multidisciplinary teams effectively. Knowledge of
CHAP standards and experience ensuring compliance with
accreditation requirements. Excellent organizational skills with
attention to detail in clinical documentation and quality assurance
processes. Ability to develop strategic plans for service
improvement and staff development. Current licensure or
certification relevant to healthcare management is preferred.
Demonstrated ability to adapt to evolving healthcare regulations
and industry best practices. This role is ideal for a motivated
