In this role, you will manage the comprehensive care for patients before, during, and after transplantation, embodying our mission of providing superior, whole-person care. As an integral part of our multi-disciplinary team, you will use your expertise to oversee patient assessments, plan treatment regimes, and ensure the seamless coordination of care across different service lines. Coordinate all aspects of care for transplant patients, including pre-operative preparation and post-operative recovery. Work alongside surgeons, physicians, and other healthcare professionals to ensure a comprehensive care plan. Educate patients and families about transplant procedures, care, and ongoing health management. Monitor patient health and respond to changes in condition promptly. Manage medication regimes, follow-up appointments, and recovery plans. Strong clinical skills in a high-acuity healthcare setting Excellent communication and interpersonal skills Ability to work collaboratively in an interdisciplinary team Detailed knowledge of transplant procedures and patient care Strong organizational and multi-tasking skills Medical Insurance Dental Insurance Vision Insurance 401k Current Registered Nurse (RN) license Certified Clinical Transplant Nurse (CCTN) preferred
