Halifax Health is seeking an Admissions Resource Specialist for
the Hospice Department.
The Admissions Resource Specialist assists with the day to day
operations of the Intake/Admissions office under the direction of
the Admissions Coordinator. Assists with the administrative and
nursing staff so department functions with a sense of urgency to
convert referrals to admission of medically eligible hospice
patients. Complies with all state and federal regulations. Assists
with the timely and accurate receipt of medical records and data
collection and review.
Provides orientation, ongoing education, guidance and serves as
a mentor to the Intake nursing professionals. Ensure appropriate
documentation and interaction with referral sources and timely
visits to initiate and complete the admission process accurately.
Provides coverage for the Admission Coordinator as assigned.
Participate in QI/UR and PIP projects.
- Registered Nurse with minimum of 2 years Hospice experience or
other leadership in health care organization.
- Current Registered Nursing License in Florida.
- Current Florida Driver's License.
- Supervisory experience preferred.
- Demonstrates critical thinking skills, excellent problem
- Must have excellent interpersonal and customer service
- Ability to work well with the Community Liaisons, hospice
teams, referral sources, patients and families.
